Submission Forms

Submission forms collect information about the samples that you submit to us for analyses. They collect basic information, background information, and the types of tests that you would like to have us do so we can better serve your needs.

Important: select the appropriate EXTERNAL form below. Only select the INTERNAL form if at least one of the following applies:

  • you are a member of the Cornell faculty, staff, students, or alumni
  • You are an employee of a Cornell Cooperative Extension Office
  • you are an employee of a New York State Soil and Water Conservation District (SWCD)
  • you are affiliated Ag Consultants

If you work for an extension office, SWCD, or work for agricultural consulting agency or are affiliated with Cornell University, you should not be purchasing soil bags at the INTERNAL discounted price and selling the soil bags at the EXTERNAL price to the growers.

Soil

The Standard Fertility Test Package includes: pH, exchange acidity (lime requirement), organic matter, and Morgan extractable phosphorus, potassium, calcium, magnesium, aluminum, iron, zinc, and manganese. It also includes a Soil Test Report with interpretations and fertilizer recommendations. However, samples submitted using the Form RS - Research Soil Analysis do not include any interpretations and fertilizer recommendations.
