Submission forms collect information about the samples that you submit to us for analyses. They collect basic information, background information, and the types of tests that you would like to have us do so we can better serve your needs. We have dozens of submission forms specific to the type of sample you want analysed, the number of samples, type of crop, purpose (i.e., commercial, homegrower, research), plus other criteria. In order that you receive the most appropriate form to fill out, we need to ask you a few questions. You will then be taken to the correct submission form.
