Submission forms collect information about the samples that you submit to us for
analyses. They collect basic information, background information, and the types
of tests that you would like to have us do so we can better serve your needs.
Important: select the "Internal" form if at least one of the following applies:
- you are a member of the Cornell faculty, staff, students, or alumni
- you are an employee of a New York State Soil and Water Conservation District (SWCD)
- you are affiliated Ag Consultants
If NONE of the above applies, please select the "External" form.
